ABSTRACT

SUMMARY OBJECTIVE: This article aims to alert health professionals for cancer screening in the face of the possibility of new waves of disease. METHODS: A narrative review was conducted through a search in MEDLINE, Lilacs, Chinese Biomedical Literature Database, and international medical societies publications. RESULTS: Breast cancer: in high-risk patients (confirmed familial cancer syndrome or with high-risk tools scores), clinicians should act according to usual recommendations; in average-risk individuals, consider screening with mammography with a longer time span (maximum of two years). Cervical cancer: women turning 25 years old who have already been immunized and with no previous Pap test can have the test postponed during the pandemic; if there is no previous dose of Human Papillomavirus vaccination, initiation of screening should be recommended following a more rigid approach for COVID prevention; in women over 30 years of age who have never participated in cervical screening, the first screening exam is also essential. Colorectal cancer: if the individual is at elevated risk for familial cancer, the screening with colonoscopy according to usual recommendations should be supported; if at average risk consider screening with Fecal Occult Blood Test. Prostate cancer: there is a trend to postpone routine prostate cancer screening until the pandemic subsides. CONCLUSIONS: The decision to keep cancer screening must be discussed and individualized, considering the possibility of new waves of COVID-19.

ABSTRACT

Resumen En la actualidad existe un gran número de apósitos dada la amplia disponibilidad de biomateriales y principios bioactivos, por lo cual se hace necesario un consenso acerca de la clasificación de estos, para no generar confusiones a la hora de entender su utilidad y su idóneo manejo en el ambiente clínico. Por este motivo, en el presente artículo se hace una revisión bibliográfica utilizando las bases de datos SCOPUS, ScienceDirect y Web of Science, con ecuaciones de búsqueda que incluían las palabras clave de los diferentes tipos de apósitos. Con esta información se encuentra que los apósitos pueden ser clasificados de acuerdo con su complejidad, la naturaleza del material polimérico, su permeabilidad, su interacción biológica con la herida y su acción terapéutica, lográndose tener una definición detallada con todos las características relevantes para hacer una adecuada elección de un apósito. Adicionalmente, se incluye una revisión acerca del proceso de cicatrización y los tipos de heridas, dado que de esto dependen los fines terapéuticos y la selección de un apósito u otro.


Abstract Nowadays, there is wide variety of dressings because availability of biomaterials and bioactive components, thus a consensus is needed on their classification, to avoid in understanding their usefulness and their proper handling in clinical practice. Therefore, in this paper a bibliographic review is made using the SCOPUS, ScienceDirect and Web of science databases, with search equations which include the keywords of different types of wound dressings. With this information, we found that dressings can be classified according to their complexity, the nature of the polymeric material, its permeability, its biological interaction with the wound and its therapeutic action, in order to have a detailed definition with all the relevant characteristics to make a proper choice of a dressing. Additionally, a review about the healing process and the types of wounds is included, since this have an important influence on the therapeutic purposes and the correct selection of dressings.

ABSTRACT

RESUMO Objetivo Avaliar a relação entre a adesão à oxigenoterapia domiciliar prolongada (ODP) e a mortalidade em pacientes com doença pulmonar obstrutiva crônica (DPOC) e insuficiência respiratória crônica e suas características clínicas. Métodos Análise retrospectiva longitudinal de 254 pacientes com DPOC e insuficiência respiratória crônica no período de 2008 a 2016. No início do estudo, avaliamos o diagnóstico, valores espirométricos, gasometria arterial, hemograma, oximetria de pulso, composição corporal e questionários de saúde (dispnéia, qualidade). vida, ansiedade e depressão). Para a análise de adesão referida ao ODP, foram incluídos 199 pacientes, divididos de acordo com a prescrição de oxigênio: 12h/dia (G1), 15h/dia (G2) e 24h/dia (G3). As causas e datas da morte foram estudadas durante o período de cinco anos. Resultados Em cinco anos, identificamos 124 óbitos (62,3%). Não houve diferença significativa na mortalidade entre os grupos de adesão (p = 0,75) e não encontramos diferenças nos parâmetros clínicos avaliados. A prescrição de ODP não foi associada à mortalidade (p = 0,07). Na análise de regressão de Cox, não houve associação entre mortalidade e não adesão ao ODP (HR: 0,75; IC95%: 0,21-2,70). O risco de mortalidade aumentou no G3 em comparação ao G1 (HR: 7,16; IC 95%: 1,44-35,38) e naqueles com maior escore de depressão (HR: 1,35; IC: 1,14-1,59). Conclusão Não foi encontrada associação entre adesão à ODP e mortalidade em pacientes com DPOC e insuficiência respiratória. Não houve diferenças clínicas entre os grupos de adesão.


ABSTRACT Objective Assess the relationship between adherence to long-term oxygen therapy (LTOT) with mortality in patients with chronic obstructive pulmonary disease (COPD) and chronic respiratory failure and their clinical features. Methods Longitudinal retrospective analysis of 254 patients with COPD and chronic respiratory failure from 2008 to 2016. At baseline, we evaluated the diagnosis, spirometry values, arterial blood gas analysis, blood count, pulse oximetry, body composition and health questionnaires (dyspnea, quality of life, anxiety and depression). For referred adherence analysis to LTOT we included 199 patients, divided according to prescription of oxygen: 12h/day (G1), 15h/day (G2) and 24h/day (G3). The cause of death and dates were studied over the five-year period. Results In five years we identified 124 deaths (62.3%). No significant difference was found in mortality between the adherence groups (p=0.75) nor did we find differences in the clinical parameters evaluated. LTOT prescription was not associated with mortality (p=0.07). In Cox regression analysis, there was no association between mortality and non-adherence to LTOT (HR: 0.75; IC95%: 0.21-2.70). The risk of mortality was increased in G3 compared with G1 (HR: 7.16; IC 95%: 1.44-35.38) and in those with a higher depression score (HR: 1.35; IC: 1.14-1.59). Conclusion No association was found between LTOT adherence and mortality in patients with COPD and respiratory failure. There were no clinical differences between the adherence groups.

ABSTRACT

Resumo Objetivo: Avaliar os resultados clínicos do Crosslinking Transepitelial (CXL) analisando sua eficácia em pacientes portadores de ceratocone progressivo. Métodos: Estudo transversal retrospectivo com 49 olhos e 37 pacientes com idades entre 10 e 50 anos, submetidos à técnica de CXL em 2017 no Instituto Panamericano da Visão, em Goiânia, Goiás, Brasil. Foi utilizado o sistema KXL Avedro programado no modo pulsado com intervalo (1/1 segundo), usando 45 mW/cm² com 7,2 J e solução de riboflavina a 0,25% da Avedro com córneas irradiadas por 8 minutos. Foram coletados os dados: sexo, idade, acuidade visual sem correção (AVSC), acuidade visual com correção (AVCC), astigmatismo, paquimetria no ponto mais fino e astigmatismo ceratométrico no pré-operatório e pós-operatório com 1, 6 e 12 meses. Foram utilizados os testes Kolmogorov-Smirnov, Qui-quadrado de Pearson, Friedman, Dunnett à posteriori e a correlação de Spearman. Resultados: Vinte e três pacientes (62,2%) eram do sexo feminino e 14 (37,8%) do sexo masculino. A média de idade foi de 27,89 (±10,89) anos. A AVSC e AVCC melhoraram significativamente no pré-operatório em relação a 1 mês (p=0,01) e (p<0,001), 6 meses (p<0,001 ambas) e 12 meses (p<0.001 ambas). O astigmatismo reduziu significativamente no pré-operatório em relação a 6 meses (p=0,02) e 12 meses (p=0,02). A paquimetria no ponto mais fino manteve-se constante no período (p=0,95). A diferença entre k2 e k1 (astigmatismo ceratométrico) mostrou redução significativa no pré-operatório em relação a 1 mês (p=0,01). Conclusão: A técnica de CXL foi segura e eficaz no tratamento e estagnação da doença em pacientes com ceratocone progressivo.


Abstract Objective: To evaluate the clinical results of Transepithelial Crosslinking (CXL) by analyzing its efficacy in patients with progressive keratoconus. Methods: Retrospective cross-sectional study with 49 eyes and 37 patients aged 10 to 50 years submitted to the CXL technique in 2017 at the Instituto Panamericano da Visão, in Goiânia, Goiás, Brazil. The Avedro KXL system was programmed in pulsed mode with interval (1/1 second), using 45 mW/cm² with 7.2 J and 0.25% riboflavin solution of Avedro with irradiated corneas for 8 minutes. Data were collected: sex, age, uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), astigmatism, pachymetry at the thinnest point and keratometric astigmatism in the preoperative and postoperative periods at 1, 6 and 12 months. The Kolmogorov-Smirnov, Pearson's Chi-square, Friedman, Dunnett, and the Spearman correlation were used. Results: Twenty-three patients (62.2%) were female and 14 (37.8%) male. The mean age was 27.89 ± 10.89 years. The UDVA and CDVA significantly improved in the preoperative period in relation to 1 month (p = 0.01) and (p <0.001), 6 months (p <0.001 both) and 12 months (p <0.001 both). Astigmatism significantly reduced preoperatively in relation to 6 months (p = 0.02) and 12 months (p = 0.02). The pachymetry at the thinnest point remained constant in the period (p = 0.95). The difference between k2 and k1 (keratometric astigmatism) showed a significant reduction in the preoperative period in relation to 1 month (p = 0.01). Conclusion: The CXL technique was safe and effective in the treatment and stagnation of the disease in patients with progressive keratoconus.

ABSTRACT

RESUMO Objetivo: Avaliar a prevalência de incapacidades físicas, cognitivas e psiquiátricas, fatores associados e sua relação com qualidade de vida em pacientes sobreviventes de internação em unidades de terapia intensiva brasileiras. Métodos: Um estudo de coorte prospectivo multicêntrico está sendo conduzido em dez unidades de terapia intensiva adulto clínico-cirúrgicas representativas das cinco regiões geopolíticas do Brasil. Pacientes com idade ≥ 18 anos que receberam alta das unidades de terapia intensiva participantes e permaneceram internados na unidade de terapia intensiva por 72 horas ou mais, nos casos de internação clínica ou cirúrgica de urgência, e por 120 horas ou mais, nos casos de internação cirúrgica eletiva, serão incluídos de forma consecutiva. Estes pacientes serão seguidos por 1 ano, por meio de entrevistas telefônicas estruturadas 3, 6 e 12 meses pós-alta da unidade de terapia intensiva. Dependência funcional, disfunção cognitiva, sintomas de ansiedade e depressão, sintomas de estresse pós-traumático, qualidade de vida relacionada à saúde, re-hospitalizações e mortalidade em longo prazo serão avaliados como desfechos. Discussão: O presente estudo tem o potencial de contribuir para o conhecimento a respeito da prevalência e dos fatores associados à síndrome pós-cuidados intensivos na população de pacientes adultos sobreviventes de internação em unidades de terapia intensiva brasileiras. Ademais, a associação entre síndrome pós-cuidados intensivos e qualidade de vida relacionada à saúde poderá ser estabelecida.


ABSTRACT Objective: To establish the prevalence of physical, cognitive and psychiatric disabilities, associated factors and their relationship with the qualities of life of intensive care survivors in Brazil. Methods: A prospective multicenter cohort study is currently being conducted at 10 adult medical-surgical intensive care units representative of the 5 Brazilian geopolitical regions. Patients aged ≥ 18 years who are discharged from the participating intensive care units and stay 72 hours or more in the intensive care unit for medical or emergency surgery admissions or 120 hours or more for elective surgery admissions are consecutively included. Patients are followed up for a period of one year by means of structured telephone interviews conducted at 3, 6 and 12 months after discharge from the intensive care unit. The outcomes are functional dependence, cognitive dysfunction, anxiety and depression symptoms, posttraumatic stress symptoms, health-related quality of life, rehospitalization and long-term mortality. Discussion: The present study has the potential to contribute to current knowledge of the prevalence and factors associated with postintensive care syndrome among adult intensive care survivors in Brazil. In addition, an association might be established between postintensive care syndrome and health-related quality of life.

ABSTRACT

Resumo A Vigilância em Saúde pressupõe olhar atento sobre a situação de saúde de populações, de modo que se compreenda a saúde, a doença e o cuidado como manifestações indissociáveis da existência humana. Considerando esta perspectiva, o artigo se propõe a examinar as práticas de saúde a partir de alguns de seus processos comunicacionais, estes marcados por uma lógica profissional-centrada, que privilegia o discurso cientificista, vertical e autoritário, predominante nos espaços do Sistema Único de Saúde. No território, o processo de comunicação é determinante. Por seu intermédio se dá a interação social e o fazer cotidiano que reterritorializam os elementos da totalidade social: homens, empresas, instituições são redimensionadas na lógica: a do localmente vivido, abrindo espaço para uma comunicação mais horizontalizada e democrática.


Abstract Supervision of a health system presupposes keeping an attentive eye on the health situation of populations, so as to understand health, illness and healthcare as indissociable manifestations of human existence. Taking this point of view, this article examines health practices from the basis of some of their processes of communication. These are markedly professional-centered in their logic, with their emphasis on scientific, vertical and authoritarian discourse, predominantly in the spaces of the Unified Health System (SUS). In the territory, the process of communication is determinant. As a result of social interaction in daily life, the communication process reterritorializes the elements of the social totality: people, companies, institutions are re-dimensioned in the logic. It is a characteristic space for activities that aim for a more horizontal and democratic flow of communication.

ABSTRACT

Cardiopulmonary bypass (CPB) with extracorporeal circulation produces changes in the immune system accompanied by an increase in proinflammatory cytokines and a decrease in anti-inflammatory cytokines. We hypothesize that dexmedetomidine (DEX) as an anesthetic adjuvant modulates the inflammatory response after coronary artery bypass graft surgery with mini-CPB. In a prospective, randomized, blind study, 12 patients (4 females and 8 males, age range 42-72) were assigned to DEX group and compared with a conventional total intravenous anesthesia (TIVA) group of 11 patients (4 females and 7 males). The endpoints used to assess inflammatory and biochemical responses to mini-CPB were plasma interleukin (IL)-1, IL-6, IL-10, interferon (INF)-γ, tumor necrosis factor (TNF)-α, C-reactive protein, creatine phosphokinase, creatine phosphokinase-MB, cardiac troponin I, cortisol, and glucose levels. These variables were determined before anesthesia, 90 min after beginning CPB, 5 h after beginning CPB, and 24 h after the end of surgery. Endpoints of oxidative stress, including thiobarbituric acid reactive species and delta-aminolevulinate dehydratase activity in erythrocytes were also determined. DEX+TIVA use was associated with a significant reduction in IL-1, IL-6, TNF-α, and INF-γ (P<0.0001) levels compared with TIVA (two-way ANOVA). In contrast, the surgery-induced increase in thiobarbituric acid reactive species was higher in the DEX+TIVA group than in the TIVA group (P<0.01; two-way ANOVA). Delta-aminolevulinate dehydratase activity was decreased after CPB (P<0.001), but there was no difference between the two groups. DEX as an adjuvant in anesthesia reduced circulating IL-1, IL-6, TNF-α, and INF-γ levels after mini-CPB. These findings indicate an interesting anti-inflammatory effect of DEX, which should be studied in different types of surgical interventions.

ABSTRACT

A separação entre Mente e Corpo, introduzida pelo dualismo cartesiano vem, gradativamente, sendo substituída por novos modelos de representação do organismo humano. A Teoria da Cognição Situada introduz paradigmas que rompem com a hegemonia do cérebro frente a outras partes do corpo e com a ideia de que os processos cognitivos ocorrem exclusivamente a partir de etapas mentais e internas. Ao apresentar a cognição como fruto do acoplamento entre organismo e ambiente, esta teoria muda o eixo de muitas questões atuais. A partir disso, são abordados temas contemporâneos, como a virtualidade e algumas produções científicas e cinematográficas recentes­ como os filmes Matrix e Avatar ­ com o intuito de aprofundar esta discussão e apresentar alguns questionamentos a essa concepção de cognição


The historical divorce beetween body and mind, introduced by Descartes's dualismo, is gradually being substituted by other models of the human organism. The theory of embodied cognition brings in paradigms that rearrange the hegemony of the brain over the body and discloses the ideia that cognition processes occur exclusively from inner and mental activities. By introducing cognition as the result of the connection between organism and enviroment, this theory changes the axle of many present issues. From that on, some contemporaneous subjects ­ like virtuality e some movies, as "Matrix" and "Avatar" - can be approached with the goal of deepen this discussion and can present some other issues linked to this conception of cognition

ABSTRACT

Este estudo teve como objetivo desenvolver e verificar as evidências iniciais de validade e a consistência interna de uma Escala de Avaliação de Estressores Psicossociais no Contexto Laboral. Participaram do primeiro estudo 630 trabalhadores (mulheres = 51,5%, média de idade = 32,77; DP = 10,33) e, do segundo, 305 trabalhadores (mulheres = 44,3%; média de idade = 34,62; DP = 12,36). As análises fatoriais exploratórias indicaram uma estrutura fatorial latente de sete fatores, com índices de consistência interna aceitáveis. Tal estrutura foi replicada na análise fatorial confirmatória. As correlações dos fatores com os afetos negativos dirigidos ao trabalho e a satisfação no trabalho foram, em geral, nas direções esperadas. Concluiu-se que a escala apresentou evidências iniciais de validade e consistência interna, o que recomenda seu uso futuro para fins de diagnóstico e pesquisa. (AU)


The present study reports the development and psychometric parameters of the Evaluation of Psychosocial Stressors in the Labour Context Scale. Exploratory factor analysis conducted in Study 1 with data from 630 employees (females = 51.5%; mean age = 32.77; SD = 10.33) suggested a seven-factor structure for the 35 items, with good internal consistency indicators for all subscales. The seven-factor structure was supported with confirmatory factor analysis in another sample of 305 employees (44.3% females, mean age = 43.62; SD = 12.36) in Study 2. The correlations between the seven factors and both negative affects towards work and job satisfaction were overall consistent with predictions. This study provides initial evidence of validity and internal consistency for the new scale. The study concludes by recommending further psychometric tests of the scale as well as its future use for diagnosis and research. (AU)

ABSTRACT

The aim of the study was to compare physical growth and body adiposity in Chilean adolescents according to the CDC -2000 and CDC-2012 reference. A numberof19,970students, between 14 and 18 years old, participated in the study. Adolescents belonged to the three units of the Chilean school education system. Weight, height and Body Mass Index were measured. The results indicated that Chilean students showed similar body weight, low height and high prevalence of overweight (28.3 to 32.5 %) according to the CDC-2000 reference. However, by the CDC- 2012 reference, results showed low weight and height, and consequently lower values of overweight (9.0 to 6.6%). We conclude that the use of a reference updates can cause confusion when analyzing the physical growth and body adiposity of Chilean adolescents. The results suggest the need for construction of a national curve which can be used to make the assessment of growth and nutritional status more reliably.


El objetivo del estudio fue comparar los parámetros del crecimiento físico y de adiposidad corporal en adolescentes chilenos en función a la referencia del CDC-2000 y CDC-2012. Se estudiaron 19970 escolares de 14 a 18 años. Los adolescentes pertenecen a las tres dependencias de escolaridad del sistema educativo chileno. Se determinó el peso, estatura y el Índice de Masa Corporal. Los resultados indicaron que los escolares chilenos mostraron similar peso corporal, menor estatura y altos valores de prevalencia de exceso de peso (28,3 a 32,5%) a través de la referencia del CDC-2000, sin embargo, por medio de la referencia del CDC-2012, mostraron bajo peso y estatura, consecuentemente valores inferiores de exceso de peso (9,0 a 6,6%). Se concluye que el uso de una referencia actualizada puede causar confusión al analizar el crecimiento físico y la adiposidad corporal de adolescentes chilenos. Los resultados sugieren la construcción de una curva nacional, con lo cual, es posible efectuar la valoración del crecimiento y estado nutricional de forma más confiable.

O presente trabalho teve como objetivo avaliar os efeitos de diferentes cargas de exercícios físicos no comportamento da frequência e do ritmo cardíaco de cães saudáveis, ao longo das 24 horas de gravação Holter. Utilizaram-se 23 cães, sem restrição de sexo, idade ou raça, dos quais 15 foram submetidos a atividades físicas e pertenciam ao grupo controle. Os resultados mostraram alterações na frequência cardíaca durante as atividades propostas, em que se alcançaram 174±27bpm durante a caminhada leve, 148±30 durante a atividade com bola e 165±26 durante o Cooper. Ademais, observaram-se arritmias, incluindo bloqueios atrioventriculares e extrassístoles e maior número e duração de pausas (até 5s) em repouso, além de uma menor variação na frequência cardíaca (FC) durante o período de sono dos animais do grupo experimental. Esses eventos não foram observados no grupo controle, o que sugere que o exercício físico extenuante interfere na modulação autonômica do coração, podendo predispor a arritmias importantes.


This study aimed to evaluate the effects of different loads of exercises on the heart rate behavior and rhythm in healthy dogs, over the 24-hour Holter recording. Twenty-three dogs were used without gender, age or breed restriction, and 15 animals were submitted to physical activities and 8 were used as control group. The results showed changes in heart rate during the proposed activities, with 174±27 bpm while walking, 148±30 during ball activity and 165±26 during Cooper. Moreover, arrhythmias were observed, including atrioventricular blocks and premature complexes and a larger number and duration of pauses (up to 5 sec) at rest, and provided a smaller variation in the HR during the sleep period of animals from the experimental group. These events were not observed in the control group, suggesting that strenuous physical exercise affects the autonomic modulation of the heart, and may lead to important arrhythmias.

ABSTRACT

PURPOSE: To correlate the central subfield thickness (CST) measured by CirrusTM SD-OCT with best-corrected visual acuity (BCVA) and structural changes in diabetic macular edema (DME). METHODS: The transversal study evaluated 200 patients with non-proliferative diabetic retinopathy (NPDR) and selected 55 eyes with DME between January, 2010 and April, 2011. Spectral domain OCT was performed in patients with type 2 diabetes and DME. CST and BCVA were correlated with the edema morphology and the ELM (external limiting membrane) integrity. Statistical tests were applied to validate the results. RESULTS: There was no difference between genders in the NPDR classification. 47.3% of the patients showed moderate NPDR. The CST average for male was of 393.58 µm and 434.16 µm for female, with no statistically significant difference. The patients with continuous ELM showed lower CST average (368.73 µm) than those with disrupted ELM (521.43 µm). There was a strong correlation between the macular volume and CST (59.63%), but poor correlation between age and CST (2.9%). Also, there was a significant difference between the average CST and the type of macular edema. Patients with serous detachment showed higher CST average (488.71 µm) than those with cystoid macular edema (CME) and diffuse edema. Patients with severe NPDR showed higher CST average (491.45 µm), if compared to mild and moderate NPDR. Cystoid macular edema was the most common type of edema (49.1%) and showed the worse VA. Patients with disrupted ELM showed worse BCVA. Patients with higher CST showed worse BCVA. There was a significant difference between the CST average of the case group (407.6 ± 113,1 µm) and the control group (diabetic patients without DME: 252 ± 12.5 µm). There was also a significant difference in the BCVA variables and macular volume between case and control groups. CONCLUSION: The study suggests that the CST of diabetic patients with edema is higher than the control group, the increase in CST of diabetic patients with edema leads to worsening of BCVA and macular volume. Continuous ELM showed lower CST average, and the serous detachment showed higher CST average. CirrusTM proved to be an important tool in the DME evaluation.


OBJETIVO: Correlacionar a espessura do subcampo central (ESCC) medida pelo CirrusTM SD-OCT com a acuidade visual (AV) e as mudanças estruturais no edema macular diabético (EMD). MÉTODOS: Um estudo transversal avaliou 200 pacientes com retinopatia diabética não proliferativa (RDNP) e selecionados 55 olhos com EMD entre janeiro de 2010 e abril de 2011. OCT spectral foi realizado em pacientes com diabetes tipo 2 e com edema macular diabético (EMD). A ESCC e a AV foram correlacionados com a morfologia do edema e a integridade da membrana limitante externa (MLE). Aplicaram-se testes estatísticos para validação dos resultados. RESULTADOS: Não houve diferença entre os sexos na classificação RDNP. 47,3% dos pacientes apresentou RDNP moderada. A média da ESCC no sexo masculino foi de 393,58 µm e no feminino de 434,16 µm, sem diferença estatística significativa. Pacientes com MLE íntegra apresentaram menor média da ESCC (368,73 µm) que aqueles com MLE descontínua (521,43 µm). Encontrou-se forte correlação entre o volume macular e a ESCC (59,63%), porém pequena correlação entre a idade e a ESCC (2,9%). Encontrou-se diferença significativa entre a média da ESCC e o tipo de edema macular, aqueles com descolamento seroso apresentaram maior média de ESCC (488,71 µm). Pacientes com RDNP grave apresentaram maior média da ESCC (491,45 µm), quando comparados à RDNP leve e moderada. O edema macular cistoide foi o tipo de edema mais frequente (49,1%) e apresentou pior AV. Pacientes com MLE íntegra apresentaram melhor AV. Pacientes com maior ESCC apresentaram pior AV. Houve diferença significativa entre a média da ESCC do grupo de casos (407,60 ± 113,05 µm) e controle (diabéticos sem edema macular: 252,0 ± 12,46 µm). Também houve diferença significativa nas variáveis AV e volume macular entre o grupo de casos e controle. CONCLUSÃO: O estudo sugere que a ESCC de diabéticos com edema é maior que o grupo controle; o aumento da ESCC de diabéticos com edema cursa com piora da AV e do volume macular. MLE contínua mostrou menor média da ESCC e o descolamento seroso mostrou maior média da ESCC. CirrusTM mostrou ser importante ferramenta na avaliação do EMD.

ABSTRACT

OBJETIVO: Avaliar a eficácia e a segurança do uso intraoperatório da mitomicina C (MMC) no tratamento cirúrgico do pterígio. MÉTODOS: Estudo de 102 pacientes (204 olhos) divididos em dois grupos, e submetidos a exérese do pterígio com rotação de retalho conjuntival e aplicação intra-operatória de MMC 0,02% por 2 minutos e a mesma técnica utilizando solução salina a 0,9% (grupo controle). Os pacientes foram avaliados no 1º, 7 º, 21º, 30º, 60º e 90º dia de pós-operatório (PO). RESULTADOS: Houve um maior acometimento do pterígio no sexo feminino (59,9%). A idade variou entre 33 a 69 anos. As recidivas (9) no no grupo MMC foram identificadas no 90º PO. No grupo controle foi identificado 18 recidivas em 60 dias de pós-operatório. As complicações identificadas no grupo da MMC foram: ceratite, edema de pálpebra, deiscência de sutura, isquemia, granuloma, corneal dellen e simbléfaro. CONCLUSÃO: O uso da MMC foi útil na redução da taxa de recidiva do pterígio, comparada ao uso de solução salina, porém seus efeitos deletérios devem sempre ser avaliados.


PURPOSE: Measure of efficacy and the security of intraoperative of mitomycin C (MMC) in the pterygium surgery. METHODS: Realized prospective analysis of 102 patients (204 eyes) divided in two groups submitted intraoperative MMC 0.02 % for 2 minutes or saline solution 0.9%. The patients were available in 1º, 7 º, 21º, 30º, 60º and 90º days of postoperative. RESULTS: The pterygium was more prevalence in females (58,33%). The age various 33 to 69 years. The recidive (9) was in the 90º PO in the group of MMC. In the control group was identificated two recidives in 60 days. The complications found in MMC group were: ceratitis, lips edema, dehiscence of suture, ischemia, granuloma, corneal dellen and simblefaro. CONCLUSION: The use of MMC was useful in reducing the rate of recurrence in pterygium compared to the use of saline solution, but its harmful effects must always be evaluated.

ABSTRACT

O estudo teve como objetivo desenvolver e validar uma escala para avaliação dos afetos dirigidos ao contextro do trabalho. A amostra foi composta por 293 trabalhadores de ambos os sexos, que responderam a uma versão inicial da escala de afetos, com 40 itens, bem como a uma escala de satisfação no trabalho e a uma escala de exaustão emocional. A análise fatorial exploratória resultou na elaboração de uma versão final da escala de afetos composta de 28 itens, distribuídos em duas dimensões - afetos positivos e afetos negativos, e índices de consistência interna de 0,93 e 0,90, respectivamente. As correlações da versão final da escala de afetos com a satisfação no trabalho e a exaustão emocional atestaram sua validade convergente e discriminante. Concluiu-se que as características psicométricas da escala de afetos no trabalho recomendam seu uso em pesquisas brasileiras destinadas a avaliar a freqüência dos afetos dirigidos ao contexto do trabalho.


The purpose of the study was to develop and validate a work affect scale. The sample was composed of 293 workers, from both sexes. They answered a 40 item first version of the affect scale, a work satisfaction scale and an emotional exhaustion scale. After the exploratory factor analysis, the final version of the affect scale was reduced to 28 items, which were divided into two factors - positive and negative affect, of internal consistency indexes of 0,93 and 0,90, respectively. The affect scale showed its convergent and discriminative validity in correlation to work satisfaction and emotional exhaustion. In conclusion, the psychometric characteristics proved to be adequate for the adoption of the affect scale for evaluating the frequency of affects in the workplace, in Brazilian studies.

ABSTRACT

Neste estudo, identificaram-se polipeptídeos associados à integridade da membrana plasmática (IMP) de espermatozóides suínos após o processo de congelamento/descongelamento. Por meio do perfil protéico do plasma seminal em SDS-PAGE, observou-se a presença de nove bandas polipeptídicas com pesos moleculares que variaram de 11,97 a 122,52kDa. Detectou-se que uma banda de 26,58kDa esteve associada à baixa IMP (<55 por cento). Não foi verificada associação entre as outras bandas e a IMP. Conclui-se que o fator polipeptídico de 26,58kDa está associado à baixa integridade da membrana plasmática do espermatozóide suíno após o congelamento/descongelamento.


Polypeptides associate to membrane integrity (MI) of swine spermatozoa submitted to freezing and thawing were identified. The protein profile of seminal plasma analyzed by SDS-PAGE allowed the identification of nine polypeptide bands with molecular weight ranging from 11.97 to 122.52kDa. One 26.58kDa band was associated with reduced MI (<55 percent). No associations among other bands and MI were observed. The 26.58kDa factor is associated with reduction of membrane integrity of swine spermatozoa after freezing and thawing.

ABSTRACT

Data on the occurrence of Yersinia species. other than Y. pestis in Brazil are presented. Over the past 40 years, 767 Yersinia strains have been identified and typed by the National Reference Center on Yersinia spp. other than Y. pestis, using the classical biochemical tests for species characterization. The strains were further classified into biotypes, serotypes and phagetypes when pertinent. These tests led to the identification of Yersinia cultures belonging to the species Y. enterocolitica, Y.pseudotuberculosis, Y. intermedia, Y. frederiksenii and Y. kristensenii. Six isolates could not be classified in any of the known Yersinia species and for this reason were defined as Non-typable (NT). The bio-sero-phagetypes of these strains were diverse. The following species of Yersinia were not identified among the Brazilian strains by the classical phenotypic or biochemical tests: Y. aldovae, Y. rhodei, Y. mollaretti, Y. bercovieri and Y.ruckeri. The Yersinia strains were isolated from clinical material taken from sick and/or healthy humans and animals, from various types of food and from the environment, by investigators of various Institutions localized in different cities and regions of Brazil.

ABSTRACT

É apresentada uma síntese sobre os peixes do Alto Paraná, com base em dados de coleções, dados de literatura e novas coletas. Trezentas e dez espécies, de 11 ordens e 38 famílias, são referidas para a drenagem, aumentando significativamente números anteriores. Dentre as espécies da área, 236 (76,1 por cento) são autóctones, 67 (21,6 por cento) alóctones e sete (2,3 por cento) exóticas. As principais causas de ocorrência de espécies não nativas (alóctones e exóticas) foram a dispersão a partir do baixo Paraná, após a construção do Reservatório de Itaipu e o escape de pisciculturas. A maior parte das espécies referidas (65 por cento) tem porte pequeno, sendo menor que 21 cm de comprimento; dentre essas, a maioria ocorre apenas em riachos e cabeceiras. Apesar da ictiofauna do Alto Paraná ser uma das melhor conhecidas e mais estudadas, o número de espécies descritas ou referidas para a área tem crescido exponencialmente, o que indica que a riqueza apresentada está longe de representar a realidade. De fato, várias novas espécies têm sido descritas nos últimos anos e cerca de 50 novas espécies, já reconhecidas, estão em fase de descrição. A melhoria no conhecimento sobre a ictiofauna do Alto Paraná é proporcional ao número de pesquisadores envolvidos em estudos na bacia e reflete, de modo inequívoco, iniciativas recentes que têm estimulado e incrementado pesquisas taxonômicas, facilitado o acesso ao material depositado em coleções científicas e aumentado as coletas em áreas e ambientes pouco amostrados. Entretanto, mantido o ritmo de descrições de novas espécies ocorrido até agora nessa última década, as 50 novas espécies já reconhecidas estariam descritas apenas dentro de dez anos, um tempo demasiadamente longo. Por essa razão é muito importante que a comunidade científica e os órgãos de fomento encontrem e viabilizem iniciativas de modo a aumentar esse ritmo de descrições de novos táxons e disponibilizar esses novos nomes mais rapidamente.


A synthesis concerning fishes from Upper Paraná River basin is presented, based on data from fish-collections, literature, and new field samples. Three hundred and ten species, pertaining to 11 orders and 38 families, are referred to the drainage, elevating anterior estimates. Concerning total species, 236 (76.1 percent) are autochthonous, 67 (21.6 percent) are allochthonous, and seven (2.3 percent) are exotic. Principal causes of occurrence of non-native species are: a) dispersal from the Lower Paraná, after the construction of Itaipu dam, and b) escapes from aquaculture farms. Most species (65 percent) are small-sized, having less than 21 cm of length, and the great majority occurs only in headwaters and small streams. One of the best known and most studied in Brazil, Upper Paraná ichthyofauna richness is far from reaching actual numbers, considering the exponential elevation of species presented herein. Indeed, in the last years various new species have been described and around 50 other species, already recognized as new, are now under description. The improvement on the knowledge about Upper Paraná ichthyofauna is proportional to the number of researchers involved with studies in the area and, unequivocally, reflects recent initiatives stimulating and incrementing taxonomic research, and also improving access to fish collections and to poor or never-sampled areas. However, if we are going to maintain the number of species descriptions per year of last decade, the 50 new species already recognized, will be described only in the next ten years, a period too long. In consequence it is very important that scientific community and grant agencies find and offer initiatives in order to elevate the number of new taxa descriptions per year.

CONTEXTO: A relação entre disfunções respiratórias e têmporo-mandibulares tem sido recentemente constatada pela incidência de dor orofacial entre indivíduos com história de doenças respiratórias obstrutivas tanto de vias aéreas superiores como nas inferiores. OBJETIVO: Avaliar comparativamente a mecânica ventilatória em indivíduos com disfunção têmporo-mandibular (DTM) e em indivíduos assintomáticos. MÉTODO: Participaram do estudo 35 voluntários, sendo 20 indivíduos com DTM e 15 assintomáticos. Os participantes foram avaliados para verificação da presença e grau de disfunção têmporo-mandibular. Realizou-se avaliação respiratória, por meio de inspeção do padrão e tipo ventilatório, medida dos perímetros torácicos, teste de força dos músculos respiratórios e histórico de enfermidade respiratória. RESULTADOS: Nos indivíduos com DTM, o modo respiratório bucal (30 por cento) e misto (15 por cento), bem como o padrão ventilatório apical (45 por cento) foram predominantes. Observou-se uma menor expansibilidade torácica e uma redução estatisticamente significante no índice de amplitude abdominal nos indivíduos com DTM. Estes indivíduos também apresentaram maior incidência de patologias respiratórias obstrutivas tanto de vias aéreas superiores (70 por cento) como inferiores (45 por cento) em relação aos assintomáticos. CONCLUSÃO: Os resultados indicam que existe uma influência da mecânica ventilatória sobre a ocorrência de disfunção têmporo-mandibular.


BACKGROUND: The relationship between respiratory and temporomandibular dysfunctions has recently been brought to light through the incidence of orofacial pain among individuals with a history of obstructive respiratory diseases in the upper and lower airways. OBJECTIVE: To comparatively evaluate the ventilatory mechanics in individuals with temporomandibular dysfunction (TMD) and in asymptomatic individuals. METHOD: Thirty-five volunteers took part in the study: 20 individuals with TMD and 15 asymptomatic individuals. The participants were evaluated to verify the presence and degree of TMD. A respiratory evaluation was carried out by means of inspecting the ventilatory pattern and type, measuring the chest perimeter, testing the respiratory muscle strength and investigating the history of respiratory diseases. RESULTS: The mouth (30 percent) and mixed (15 percent) breathing types and the apical ventilatory pattern (45 percent) predominated in individuals with TMD. There was less chest expandability and a statistically significantly smaller abdominal amplitude index in individuals with TMD. These individuals also presented higher incidence of obstructive respiratory diseases in the upper (70 percent) and lower (45 percent) airways, in relation to the asymptomatic individuals. CONCLUSION: The results indicate that there is an influence from ventilatory mechanics on the occurrence of temporomandibular dysfunction.

ABSTRACT

Peginterferon alfa plus ribavirin is currently the treatment of choice for chronic hepatitis C. Peginterferon alfa-2a (40KD) plus ribavirin has given an overall sustained virological response of 18 percent in F3/F4 previous nonresponder US patients. We evaluated the effectiveness of peginterferon alfa-2a (40KD) plus ribavirin in Brazilian patients who were relapsers or nonresponders to previous interferon-based therapy. One-hundred-thirty-four patients with biopsy-proven chronic hepatitis C, HCV RNA positive, elevated ALT and who were either relapsers (n=37) or nonresponders (n=97) to at least 24 weeks of conventional interferon/ribavirin therapy were retreated with peginterferon alfa-2a (40KD) 180mg/qw and ribavirin 800mg bid for 48 weeks. Efficacy was assessed as virological response (defined as undetectable HCV RNA) at the end of treatment (EoT) and at the end of follow-up (SVR - Sustained Virological Response). Safety assessments consisted of clinical and laboratory evaluations. In the patient sample, 72 percent were genotype 1 and 34 percent were cirrhotic. In an intention-to-treat analysis, relapser patients showed 78 percent EoT response and 51 percent SVR. Nonresponders showed 57 percent EoT response and 26 percent SVR. Positive predictive factors of SVR were non-1 genotype and relapser state. Six percent of the patients interrupted treatment because of adverse events and 45 percent had dose reduction (mainly associated with leucopenia and anemia). Brazilian patient relapsers and nonresponders to conventional interferon and ribavirin treatment can achieve a sustained virological response when retreated with peginterferon alfa-2a (40KD) and ribavirin. The safety profile is similar to that of naive patients.
